How do I place an order?
Healthcare Professionals can order with established NET 30 terms or Prepayment/Credit Card.
Patients can order directly, via Prepaid/Credit Card, with a prescription. However, please note that AMT does not process medical billing claims.
- US Direct Customers: We will need a copy of your US prescription.
- Canadian Direct Customers: We will need a copy of your Canadian prescription.
- All Other International Direct Customers: We will need a copy of your prescription from a US doctor and can only ship to countries we are registered in. Contact your local rep or AMT’s International Customer Service team to learn about how you can set up an account and order AMT products.

Why do I need a prescription to purchase an AMT device?
As a medical device manufacturing facility, AMT is required by governing bodies, like the FDA, to have a prescription on file in order to send products directly to patients.
Does AMT accept medical coverage for device purchases?
Applied Medical Technology, Inc. (AMT) does not submit insurance claims, nor are we enrolled or planning to enroll to be able to submit claims.
